ICLS is a specialised mental health commissioned funded program designed to support people aged 18 to 85 living with a severe mental illness. It offers up to two years of personalised support (plus 12 months if needed), helping you build the skills and confidence to live well in your home and community.
Support is flexible and designed to fit your life, whether you need help maintaining your home, managing your mental health, connecting with others, or pursuing education or work.

We’ll work alongside you to create a Recovery Plan that outlines your personal goals. Your Recovery Plan will evolve and grow as you do, and our team is here to work alongside you as part of your recovery journey.
At Avivo, we walk alongside you – coaching, guiding, and supporting you towards recovery. Our approach is hopeful, proactive, and shaped by your goals, whether you’re rebuilding confidence, reconnecting with community, or simply finding your feet again.
Together, we’ll co-create a personalised Recovery Plan that reflects what matters to you – your strengths, your needs, and your vision for the future. We listen deeply, value your voice, and support your autonomy every step of the way.
Working closely with your family, clinicians, and wider support network, we take a future-focused, practical, and consistent approach, always grounded in genuine human connection.

Accessing the Individualised Community Living Strategy (ICLS) is a collaborative process designed to ensure you receive the right support. This involves:
A public or private mental health service submits a referral to the WA Mental Health Commission (MHC) on your behalf.
The Mental Health Commission reviews and approves the referral for an ICLS support package upon program vacancy.
The Mental Health Commission will call for nomination to a distribution list. Public and private mental health practitioners can join this list by emailing iclsreferrals@mhc.wa.gov.au.

If you’re new to support services, exploring your options, or considering a change in provider, Avivo can guide you through the process.
