Support at Home providing aged care support with more choice and independence.
What is Support at Home?
Support at Home is Australia’s new aged care program that launched on 1 November 2025. It replaces Home Care Packages and Short-Term Restorative Care, making it easier to get the right support, when and how you need it.
This government-funded programme is all about helping older Australians stay safe, independent, and connected — in the comfort of their own homes and communities. Whether it’s help around the house, support with mobility, or more complex care, services are flexible and tailored to what matters most to you.
Support at Home funding levels
There are eight levels of funding, ranging from around $11,000 to $78,000 per year, depending on the level of support you need.

Who is eligible?
You may be eligible for a Support at Home if you:
- Are aged 65 or older (or 50+ if you’re Aboriginal or Torres Strait Islander)
- Live at home and need ongoing or more complex support

Financial hardship support
If you’re worried about the Support at Home contributions, you can apply for financial help by completing the Financial Hardship form.
Once you are approved for Support at Home, if you think you are going to struggle with the contributions you can complete the application form while waiting for allocation.
Self-funding
You don’t have to wait to get help. If you’re waiting for a package to be assigned, we can provide self-funded support to get you started sooner.

What is the Support at Home program?
This new government-funded aged care program launches on 1 November 2025, replacing both Home Care Packages (HCP) and Short-Term Restorative Care (STRC). It’s designed to be simpler, more flexible, and easier to access across Perth and regional WA.
I already have a Home Care Package—what happens to me?
Nothing changes on your end. You’ll automatically transition to the new program with your funding level maintained or improved, and your services will continue without interruption. Rest assured, the government guarantees no financial disadvantage.
What’s actually changing under Support at Home?
- More personalised funding levels: Increased from 4 to 8 levels, ranging between $11,000 and $78,000 per year.
- Quarterly budgets across three key areas:
- Clinical care (nursing, allied health)
- Independence support (personal care, mobility, AT-HM)
- Everyday living (cleaning, meals, gardening)
- Short-term services now include:
- Restorative care: up to 12 weeks
- Home modifications & assistive tech: up to $15,000
- End-of-life care: up to $25,000
- Faster access to services, with wait times targeted at three months by 2027.
Will I need to pay for my care?
Contributions vary based on your financial situation—full or part pensioner, or self-funded retiree. Income- and assets-based fees apply, with continued protections for pensioners.
Can I still choose my provider?
Absolutely! You can continue with Avivo—or select any provider you prefer. You control the choice, and we’ll help tailor services to your life, home, and community.
Is self-management an option?
Yes—if you’d like to:
- Choose your support workers
- Schedule your services
- Manage your own budget
You’re free to self-manage. But if you want help coordinating or planning, Avivo is here to support you.
What happens if my needs change?
We’ve got you covered. Simply request a reassessment, and your support level can adjust to match your changing needs.
How do I apply or transition?
- Already on a Home Care Package? No action needed—you’ll transition automatically.
- New to aged care? Apply via My Aged Care, and we’ll guide you through the assessment, funding, provider choice, and care planning process.
