The Restorative Care Pathway is a short-term program designed to help older Australians maintain or regain their independence after illness or injury. As part of the Support at Home program, it offers up to 16 weeks of coordinated clinical services, including nursing and allied health support, to assist you in achieving your personal goals.
Funding is approximately $6,000 for the 16-week period, with the possibility of an additional $6,000 if extra services are required.

Get assessed for the Restorative Care Pathway
Choose Avivo as your provider
Develop a personalised plan with a Service Coordinator
Receive support from familiar, local workers
Regular reviews to ensure your goals are being met
Note: If you’re currently receiving Short-Term Restorative Care, your provider will assist in transitioning your services to the Support at Home program.
The government covers most of the cost, with some participants asked to contribute based on income and assets.
