What Is the Restorative Care Pathway?

The Restorative Care Pathway is a short-term program designed to help older Australians maintain or regain their independence after illness or injury. As part of the Support at Home program, it offers up to 16 weeks of coordinated clinical services, including nursing and allied health support, to assist you in achieving your personal goals. 

What’s included?

Help with daily tasks like meal preparation and shopping

Personal care assistance like bathing and dressing

Support to reconnect with your community and routines

Allied health services like physiotherapy and occupational therapy

Nursing care

Education and skill-building programs focused on maintaining functional abilities

Funding is approximately $6,000 for the 16-week period, with the possibility of an additional $6,000 if extra services are required. 

Who is eligible?

You may be eligible if you:

  • Are aged 65 or older (or 50+ if Aboriginal or Torres Strait Islander)
  • Live at home
  • Need short-term support to maintain or regain independence

    Get assessed for the Restorative Care Pathway

  3. 03

    Choose Avivo as your provider 

  4. 04

    Develop a personalised plan with a Service Coordinator

  5. 05

    Receive support from familiar, local workers

  6. 06

    Regular reviews to ensure your goals are being met

Note: If you’re currently receiving Short-Term Restorative Care, your provider will assist in transitioning your services to the Support at Home program. 


Contributions to restorative care 

The government covers most of the cost, with some participants asked to contribute based on income and assets.
