Coming home after a hospital stay can feel overwhelming, especially as we get older. Avivo makes it easier, with compassionate, reliable and personalised post-hospital care designed for you or a loved one, to recover comfortably and safely in your own home.
Whether it’s after surgery, illness, or an injury, our experienced team works closely with you, your family and your healthcare providers to ensure a smooth transition from hospital to home.
Assistance with showering, dressing and grooming
Help with medications, wound care and recovery routines
Light cleaning, laundry, shopping and meal preparation
Safe transport to medical appointments, rehabilitation and community activities
Professional care for wound management, medication, health monitoring and recovery support
Assistance with mobility, exercises and therapy programs though our Allied Health network
Emotional support and company to help reduce stress and anxiety
Working with your doctors, nurses and care team to ensure your needs are met
Post-hospital care can be included as part of your funded services under a Support at Home.
If you can’t wait, or not eligible for funding you can self-fund your post-hospital care.
