Whether it’s after surgery, illness, or an injury, our experienced team works closely with you, your family and your healthcare providers to ensure a smooth transition from hospital to home.

Safe transport to medical appointments, rehabilitation and community activities
Professional care for wound management, medication, health monitoring and recovery support

Post-hospital care can be included as part of your funded services under a Support at Home.
