The End-of-Life Pathway is part of the Support at Home program. It provides extra care and support in the final months of life, helping people remain at home, surrounded by comfort and loved ones.
Support may include personal and clinical care, equipment to make daily life easier, emotional support for families, and coordination with your GP or palliative care team. At Avivo, we’ll work with you to tailor the care to your wishes.
This pathway is available to people who have been assessed as being in the last 12 months of life and who wish to stay at home with the right support. An Aged Care Assessment Team (ACAT) helps determine eligibility.
The program is funded by the Australian Government. Depending on your financial situation, you may be asked to make a small contribution, but most of the support is covered.
Support is designed to start as soon as possible once eligibility is confirmed. We’ll help make sure equipment, care, and guidance are put in place without delay.
