End of Life Pathway

The End-of-Life Pathway is part of the Support at Home program, created to help people spend their final months where they feel most at peace — at home, surrounded by the people and things they love.
With the right care, equipment, and support, you and your family can focus on moments that matter most.

What support is available?

Through the End-of-Life Pathway, Avivo can walk alongside you and your family with: 

  • Personal and clinical care to manage comfort and symptoms 
  • Equipment and home adjustments to make care easier and safer 
  • Guidance and reassurance for family members providing care 
  • Coordination with GPs, specialists, and palliative care teams 

We also offer our dedicated Death and Dying program (link to this landing page) to support individuals and families with compassionate, holistic care through the end-of-life journey. 

Who is eligible?

You may be eligible if you:

  • Have been assessed as being in the last 12 months of life
  • Wish to remain at home with the right supports in place

Are there contributions to the End-of-Life Pathway? 

Most of the cost is covered by the Australian Government. In some cases, you may be asked to make a contribution based on your income and assets. If contributions apply, they are kept as low as possible so you can focus on what matters most.

FAQ’s – End of Life Pathway  

What is the End-of-Life Pathway?

The End-of-Life Pathway is part of the Support at Home program. It provides extra care and support in the final months of life, helping people remain at home, surrounded by comfort and loved ones.

What kind of support can I expect?

Support may include personal and clinical care, equipment to make daily life easier, emotional support for families, and coordination with your GP or palliative care team. At Avivo, we’ll work with you to tailor the care to your wishes.

Who can access the End-of-Life Pathway?

This pathway is available to people who have been assessed as being in the last 12 months of life and who wish to stay at home with the right support. An Aged Care Assessment Team (ACAT) helps determine eligibility.

Does it cost anything?

The program is funded by the Australian Government. Depending on your financial situation, you may be asked to make a small contribution, but most of the support is covered.

How quickly can support begin?

Support is designed to start as soon as possible once eligibility is confirmed. We’ll help make sure equipment, care, and guidance are put in place without delay.

Can my family be involved in my care?

Absolutely. Families often play an important role, and we provide guidance, training, and reassurance so they feel confident and supported too.
