Whether you’re applying for yourself or someone you care about, starting your aged care journey can feel overwhelming. We’re here to make it easier, and walk alongside you from the very beginning.
If you’re aged 65 or over (or 50+ for Aboriginal and Torres Strait Islander people), you may be eligible for government-funded aged care services.
You can check your eligibility using the https://www.myagedcare.gov.au/eligibility-checker or by calling 1800 200 422.
Most government-funded aged care support begins with an assessment.
You can start the process by contacting My Aged Care on 1800 200 422 or visiting their website. They’ll ask a few questions to understand your needs and organise an in-home or phone-based assessment.
Avivo Tip:
🧡 Not sure what to say or how it works? Give us a call first, we’re happy to talk you through what to expect.
After your assessment, you’ll find out what type of support you’re eligible for.
We’ll help match your needs with the right type of care:
| Support Type | Best For | What You Receive |
|---|---|---|
| CHSP – Commonwealth Home Support Program | A little extra help | Occasional services like cleaning, transport or meal preparation |
| SaH – Support at Home | Ongoing or more complex needs | A personalised care plan with regular support from our team |
| Self-Funded Care | Want to start now | Immediate support with full flexibility – no waitlists |
Avivo Tip:
🧡 If you’re not sure which support is right for you, we can explain the differences and help you get started while you wait for funding.
After your assessment, you’ll receive a letter confirming your eligibility and the type of support you’ve been approved for. If you’re approved for Support at Home, you’ll be placed in a national queue based on your priority level.
Wait times vary — urgent cases may be assigned funding within a month, while others may wait 6–18 months
Once funding is available, you’ll receive a referral code. You can then choose a provider that suits your lifestyle, values, and care needs.
Things to consider:
At Avivo, we offer a wide range of aged care services, from help at home to nursing, therapy, and social support.
Your Avivo Service Coordinator will work with you to build a care plan that reflects your goals, preferences, and daily needs.
Once everything’s in place, your chosen Support Worker will begin delivering your services – in a way that’s respectful, flexible, and built around you
Your needs may change over time – and that’s okay. At Avivo, we’ll help you review your plan regularly and make adjustments so your support continues to fit your life.
We know navigating aged care can feel overwhelming, especially when you’re looking out for someone you love. Our team is here to listen, guide, and support you at every step. Whether you’re brand new to aged care or just need someone to help explain the next step, we’ll make it feel simpler.
If you’re over 65 (or 50 for Aboriginal and Torres Strait Islander people), you may be eligible for government-funded aged care services through an Aged Care assessment. What you receive depends on your needs, but there are three main ways to access support:
Commonwealth Home Support Program (CHSP) Entry-level support to stay independent | Support at Home (SaH) More ongoing or complex care, tailored to you. | Self-Funded Care Get started right away — no waitlists. |
|---|---|---|
| CHSP is ideal if you’re managing day-to-day but could use a little help around the home. It offers a small number of support hours each week for services like cleaning, gardening, transport, or meal preparation. Best for: Seniors who are mostly independent but need a little extra support. How to access: Apply through My Aged Care for an assessment. | A Support at Home plan provides more regular and coordinated support for those who need help with everyday tasks, personal care, or more complex needs like nursing, therapy, or recovery after a hospital stay. Best for: People needing consistent, flexible support to remain safely at home. How to access: Apply through My Aged Care, and receive a funding level based on your care needs. | If you’re not eligible for government funding or want to begin services immediately, self-funded care gives you full flexibility and control. You choose the services you want and pay as you go. Best for: People waiting for funding, needing short-term help, or wanting full flexibility. How to access: Contact Avivo directly and we’ll set it up with you. |
